Welcome to Grace Behavioral Health, LLC! We look forward to meeting you at the time of your first appointment. Grace Behavioral Health is an outpatient mental health office staffed by psychiatric nurse practitioners who assess, diagnose and treat individuals with psychiatric disorders. We currently treat patients 12 years of age and older. We accept most commercial insurance, Medicare and self-pay. We are currently not in network with Medicaid. We are located in beautiful downtown Owensboro at 100 W. 3rd St., Suite 305, across from Colby’s restaurant in the TRUIST (former BBT) building.
If you have received psychiatric care previously we request you have those records forwarded to us or bring them to your initial appointment.
We ask that you telephone us at least 24 hours in advance if you must cancel, so we can schedule other patients who request to be seen. Please plan on arriving 30 minutes early to your initial appointment, as there will be paperwork to complete unless this was done online prior to your appointment. Should you arrive more than 10 minutes late, your appointment may need to be rescheduled in an effort to see other patients in a timely manner. We realize your time is valuable and we want to give you prompt care if at all possible. If you do not call 24 hours in advance to cancel an appointment, your appointment will be considered a “no show” and documented in your chart (extreme circumstances will be excused). If you have 2 or more “no show” appointments, you will be terminated from the practice.
Monica Johnson’s Credentials: Monica Johnson, APRN has a Bachelor’s degree in nursing from Middle Tennessee State University, and a Master’s degree in nursing from the University of Southern Indiana. She is a nurse practitioner specializing in family psychiatric mental health including medication management. She is a member of Sigma Theta Tau International Honor Society of Nursing. She has been in the nursing profession for over 25 years. Prior to becoming a psychiatric nurse practitioner she worked as a registered nurse in multiple areas including, cardiothoracic surgical recovery, intensive care, neurology, infection control, and case management. She is board certified as a family psychiatric mental health nurse practitioner by the American Nurses Credentialing Center. She is also an avid University of Kentucky fan and alumni where she obtained her Bachelor’s of Science in Accounting prior to entering the nursing profession.
Amber Survant’s Credentials: Amber Survant, APRN has a Bachelor’s degree in nursing from Western Kentucky University, and a Master’s degree in nursing from the University of Southern Indiana. She is a nurse practitioner specializing in family psychiatric mental health including medication management. She has been in the nursing profession for 10 years. Prior to becoming a psychiatric nurse practitioner she worked as a registered nurse in women’s surgery, pediatrics, ortopeadics, and mother-baby nursing. She is board certified as a family psychiatric mental health nurse practitioner by the American Nurses Credentialing Center.
Medication refills: We ask that you contact your pharmacy for all medication refills as they will notify us via our computer refill system. Please give 72 hour notice for all medication refills with the exception of benzodiazepines (valium, klonopin, ativan) and stimulants (adderall xr, vyvanse, concerta, etc) we request 7 days notice for these medications.
After hours coverage: Our phone hours are 8:00-12:00 p.m. and 1:00-3:00 p.m., Monday thru Thursday. During regular office hours, someone will be available to answer your call as promptly as possible if you call our office number (270) 240-2320.
